Jonathan Pisani Obituary, Man recalled to prison died in cell at Forest Bank just nine days free

Jonathan Pisani Obituary – Jonathan Pisani, aged 46, was discovered deceased in his cell at HMP Forest Bank after just nine days of being released from prison. He had been convicted of firearm offenses in August 2021 and was sentenced to 15 months in prison. Despite being released on January 4, 2022, he was recalled to prison on January 13 for breaching his license conditions and was sent back to Forest Bank in Pendlebury, Salford.

According to a report by the Prisons and Probation Ombudsman (PPO), Mr. Pisani, who had a history of drug and alcohol abuse, passed away six days after he arrived at the prison on January 19, 2022. The report mentioned that his cellmate had fallen asleep after smoking ‘spice’ the night before, but Mr. Pisani did not consume any illicit substances that evening or during their time sharing a cell.

Mr. Pisani was found unresponsive in his cell at 7.05 am the next morning and was later pronounced dead by a GP. The post-mortem examination revealed that his cause of death was drug toxicity. The report also stated that Mr. Pisani had been receiving alcohol and drug detoxification treatments since his arrival at Forest Bank on January 13.

Both the HM Chief Inspector of Prisons and the Independent Monitoring Board confirmed that the substance misuse treatment at Forest Bank was satisfactory, with no issues identified in the care provided to Mr. Pisani. The substance misuse team at the prison visited him twice daily to monitor his withdrawal symptoms and administer methadone as needed. He was appropriately identified as requiring alcohol detoxification upon his intake to the facility.
